Format A5, Querformat. Vorderseite: Adressat: "Herrn // Kurt // Gremler // Jastrow // Deutsches Reich". Oben links Stempelabdruck des Absenders: "Deutsche Kriegergräber-Abteilung // beim K. und k. Kreiskommando Zamosz // Tgb. Nr. 1055 K.". Unten links Vermerk: "Dienstsache". Oben rechts schwarzer Stempelabdruck für die Briefbeförderung: mittig: "-1.XII.17", Umschrift: "[...] ZAMOSC". Sowie mittig mit violettem Buntstift: "Post z Krunk?" mit blauem Bundstift durchgestrichen.
