Die Plakette ist schildförmig gestaltet. Sie hat einen weißen Hintergrund, auf der in blau aufgedruckt ist: "Kreis- // Heimat- // Fest // Wolmirstedt // 27.6-29.6. // 1958"
Um die Plakette herum ist ein schmaler blauer Rand.
Oben am Rand ist ein kleines Loch, zum einfädeln eines Bandes.
Auf der Rückseite aufdruckt: "Nr. 12929" handschriftlich mit blauer Tinte: "9930" sowie mit Bleistift "2013 neu"
